Possible interaction between heparin and a sulphonylurea a cause of prolonged hypoglycaemia?

چکیده
Reports to the Committee on Safety of Medicines show that skin reactions are the commonest adverse reaction to trimethoprim; the committee has had a few reports of cholestatic jaundice due to trimethoprim and to trimethoprim-sulphamethoxazole. Manufacturers of trimethoprim (Duphar) have received four reports of hepatic reactions: hepatitis (two), jaundice (one), and cholestatic jaundice (one). In previous reports similar hepatic reactions to combination antibiotics have been attributed to the sulphonamide component. I suspect that some of these reactions were due to trimethoprim. The mechanism for such cholestasis is obscure as trimethoprim interferes primarily with the metabolism of folic acid. As the reaction occurred late in life in this patient and was clearly related to exposure to the drug it is unlikely to have been any other form of benign cholestasis.

OBJECTIVE To determine the association between use of sulphonylureas and risk of hypoglycaemia in relation to renal function and sulphonylurea metabolic group compared with use of metformin. DESIGN Population based cohort study using routinely collected data from general practices in England. SETTING Clinical Practice Research Datalink (CPRD) database, 2004-12. PARTICIPANTS 120 803 ne...
متن کاملHyperinsulinaemic hypoglycaemia due to chlorpropamide-induced nesidioblastosis.
A 25 year old woman suffering from recurrent attacks of hypoglycaemia underwent a laparotomy for suspected insulinoma. No tumour was found, but histology showed islet cell hyperplasia and nesidioblastosis. Although these changes have been reported as a cause of hypoglycaemia in infants, they are only rarely the cause of hypoglycaemia in adults; in the present case they were found to be the resu...

AIMS To compare the incidence of symptomatic hypoglycaemia in fasting Muslim patients with type 2 diabetes treated with sitagliptin or a sulphonylurea during Ramadan. METHODS Patients with type 2 diabetes (age ≥ 18 years) who were treated with a stable dose of a sulphonylurea with or without metformin for at least 3 months prior to screening, who had an HbA(1c) < 10% and who expressed their i...
متن کاملPrevalence of factitious hypoglycaemia associated with sulphonylurea drugs in France in the year 2000.
Hypoglycaemia presents important diagnostic and therapeutic problems. Severe and repetitive hypoglycaemic episodes without treatment may be difficult to explain. Failure to identify factitious hypoglycaemia may lead to laparotomy or pancreatectomy [1, 2]. To estimate the prevalence of factitious hypoglycaemia due to sulphonylurea drugs in patients with unexplained severe hypoglycaemia, we condu...
